Treatment alterna- tives can be eclectic, including medical or surgical intervention, voice therapy, psychological counseling, or a combi- nation approach.
When nod- ules do not respond to therapy in the patient who has been compliant with voice therapy, surgical management may be required, followed by postsur- gical voice rehabilitation.
Cysts do not respond to behav- ioral voice therapy and the definitive treatment is surgical excision, usually from a superior and lateral approach to avoid scarring of the medial edge of the vocal fold.
Surgical treatment for polypoid degeneration and Reinke's edema is common, using a lateral incision to extract the superfluous contents while protecting the medial edge of the vocal fold.3 Voice therapy is valuable both preoperatively in identifying the causes of the pathology and postoperatively for reestablishing good vocal hygiene and improved voice production.
If the tissue around the lesion is abnor- mal (called "atypia"), these lesions may signal early carcinoma.30 Voice therapy often can help restore voice quality and ensure that the patient modifies any behaviors that contributed to the epi- thelial changes.
